There was a time when doctors spoke to patients about their illnesses.  It is difficult to imagine now, but doctors used to actually ‘shoot the breeze’ with their patients during appointments.  Modern medicine has brought amazing technologies, but the systems now in place consider human interactions, teaching, and ‘shooting the breeze’ to be irrelevant to medical care.  The change has reduced the quality of care in several ways:  First, patients leave appointments confused, and confused patients are less compliant with their medications.  Second, going to the doctor used to impart a feeling of ‘being cared for’ (which went a long way toward treatment of the primary illness)—when did you last leave a doctor’s office feeling ‘cared for’?  Finally, some symptoms take time to present in an interview—particularly psychiatric symptoms, but also embarrassing sexual symptoms or incontinence—but now, by the time the patient gets up the nerve, the appointment is over and the patient is being moved toward the door.
